We provide more help during a health crisis and for chronic care. Unlike other insurance companies, we work side by side with providers to understand exactly what care is needed and when. We’re working together every step of the way.

Your Innovation Health membership means that when your health care needs stray from routine PCP visits and flu shots, we’ll step in to help. We know that while members are dealing with complex health issues; they will need our help. Because unless that member is an expert in medicine, insurance, and understands all the services available to them, there’s a good chance there’s some sort of gap in their care.

So, we’ve hired experienced professionals for you, an Innovation Health Nurse Concierge. The Nurse Concierge reviews the records of members who are showing signs of distress like multiple visits to the ER, or several behavioral health issues, or multiple prescriptions. It takes the training and careful eye of the Nurse Concierge to look at electronic health records and claims for each member and find actionable insights. 

And, then there are times we have to take it to a whole new level. Some members need more than a Nurse Concierge. Their needs are so great and so complex, the Nurse Concierge accesses a multidisciplinary group of health care professionals including a medical director, pharmacist, case managers, social workers, and other specialists. Together with the Nurse Concierge, this team can help members in getting the care they need.

Along with needing the most help, the sickest of our members represent 60% of the cost, with some portion of that being impactable…meaning if we take better care of them, costs will go down as well.  And that’s exactly what we’re doing.
